Membership with Radical Healing is OPTIONAL and we recognize these plans may not fit everyone’s needs. This is a 6-month pilot program and hopes to provide a cushion and support to folx who are concerned about losing access to health insurance in 2026 or who have been underinsured. We welcome feedback throughout the process to help us build a sustainable program.

This pilot primary care membership provides participants with reliable access to foundational medical care.

Membership includes one annual 40-minute physical and three 20-minute problem-focused visits (standard rate: $175) per year.

Therapy ONLY Subscription

This therapy pilot offers weekly or bi-weekly therapy through a subscription model.

Sessions are valued at $100+ each, (discounted from $175) and subscribers receive four sessions per month, which must be used within a six-week window to allow for travel, holidays, or schedule interruptions.

The plan uses a sliding scale to increase access. Additional sessions can be scheduled for $125.

This membership is designed for clients who have successfully matched with a provider and would like to continue their services.

Currently, the subscription plan is only available for weekly therapy sessions only.

Pelvic Floor Physical Therapy ONLY Bundle

$600 – $750 bundle for five (5) 40-minute pelvic floor physical therapy sessions

This Pelvic Floor Physical Therapy offering provides structured, short-term care through a bundled subscription model designed to support continuity, affordability, and lasting health outcomes.

All five sessions must be used within three calendar months of payment to support therapeutic momentum while allowing flexibility for travel, holidays, or scheduling interruptions.

Pricing:
$600 – $750 total paid upfront for 5 sessions ($120 – $150 per session; discounted from $175 per session). Additional visits are available for $125 per visit.

One session credit will be deducted for each late cancellation/no-show after payment for your bundle.

Ideal For: Clients seeking structured pelvic floor rehabilitation with predictable costs, significant per-session savings, and continuity of care within a trauma-informed, affirming clinical setting.

Pilot Program Notice:
All subscription models are pilot studies, and each program may be adjusted, paused, or ended at any time. Participation does not guarantee continuation beyond the pilot period. These pilots are intended to run for no more than six months, and no commitment is made to extend services beyond the pilot window.

Minimum Commitment:
All participants must agree to a minimum six-month commitment to ensure program stability and sustainability. At any point if you decide to terminate your plan, we cannot issue refunds from previous payment periods.

Emergency / Additional Visits:
Emergency or urgent same-day visits outside the included membership benefits will be billed at a flat supplemental rate of the aforementioned amount per visit. If this cost is not affordable, we will provide a list of community-based low-cost referral options to ensure the patient can still access appropriate care.

Non-Urgent Feedback or Membership Concerns:
Members with non-urgent concerns related to the subscription, billing questions, or general feedback should submit the designated form here or email membership@radicalhealing.us. A team member will respond within 72 business hours unless otherwise stated. For immediate scheduling concerns for Primary Care or Gender Affirming Care call 919-341-6111.

Provider Availability, Scheduling & Continuity of Care:
If a provider leaves the practice, becomes unavailable, or if there are scheduling disruptions, we will make every reasonable effort to provide a comparable replacement or alternative solution in a timely manner. While continuity cannot be guaranteed, maintaining stable access is a priority for this pilot. 

Session & Visit Requirements:

  • Included visits or therapy sessions must be used within the defined timeframes for each plan.
  • Unused sessions do not roll over unless explicitly stated (e.g., the therapy plan’s six-week usage window).

Cancellations, No-Shows & Medicaid-Specific Scheduling Policies:

  • Excessive cancellations may result in scheduling limitations, including same-day booking only for members who repeatedly cancel appointments.

Payment, Lapses, and Restart Fees:
The card on file will be charged the agreed-upon amount on the first of each month, with the exception of holidays. A payment lapse occurs when the subscription payment cannot be processed and the subscriber does not respond to contact attempts for 7 days.

  • A simple card decline is not considered a lapse unless unresolved for 7 days.
  • Billing will reach out to the client. 
  • After a confirmed lapse, a $75 restart fee is required to resume the membership.

Not Insurance:
These subscriptions are not insurance and do not replace or function as health insurance. They grant access only to the specific services described within each plan. Any care received outside the practice or not included in the plan’s stated benefits is the member’s responsibility. While some commercial and medicaid insurance plans may reimburse for membership plans, there is no guarantee that these services will be reimbursed by your insurance if you decide to seek reimbursement from insurance. 

Right to Modify or Terminate Pilot Services:
Because these are pilot programs, the organization reserves the right to adjust pricing, capacity caps, service availability, or program structure, or to discontinue the pilot entirely, at any time.

In the unlikely event that we terminate the pilot program in less than six months, we will prorate your remaining unused membership benefits and refund your unused portion.

Because memberships are not billed through insurance, your personal health information stays within the Radical Healing care team instead of being shared with an insurance company. This means diagnostic codes, visit details, and care decisions are not sent to third-party systems for approval or record-keeping. Members have greater control over who sees their information, which can feel especially important for people seeking gender affirming care, mental health support, or any services that are often scrutinized by insurers.

These plans are designed to help people who may lose insurance in 2026 or whose current insurance does not meet their needs. The pricing structure is often more manageable than paying out-of-pocket for care through insurance.

What are these memberships?

These memberships are pilot programs that offer predictable, subscription-based access to primary care, gender affirming care, and therapy. They are designed to help people who are underinsured, at risk of losing insurance, or seeking more reliable access to affirming care. This pilot program is intentionally limited in scope as we work to balance expanded access with high-quality, sustainable care, and additional membership options or services may be introduced in the future based on what we learn from this pilot.

Are these memberships insurance?

No. These memberships are not insurance and do not replace or act like insurance. They provide access only to the services listed in each plan. Any care outside Radical Healing, or outside the plan’s scope, is the member’s responsibility. We are not issuing superbills at this time, those who have insurance will be asked to sign an insurance waiver form. Members will receive a Good Faith Estimate contract for subscription plans.

Who are these plans designed for?

They are ideal for people who want consistent access to care without dealing with insurance restrictions. They can also help people who have high deductibles, gaps in coverage, or concerns about losing insurance in 2026.

Which providers are participating?

Only certain providers at Radical Healing are part of this smaller pilot launch. If your preferred provider is not participating at this time, please check back in mid-2026 for updates on expanded availability.

How long is the commitment?

All members agree to a six-month minimum commitment. This helps the program remain sustainable and ensures predictable access for all participants.

How many visits are included?

The number of visits depends on the plan. Each option includes a set number of primary care, gender affirming care, or talk therapy visits per year or per month. Additional visits can be purchased at a discounted rate.

What happens if I need care beyond what the plan covers?

Extra visits are available at a discounted rate. Urgent same-day visits are charged at a clear flat fee. If that fee is not affordable, Radical Healing will provide referrals to low-cost community options.

How private is this membership?

Memberships offer a higher level of privacy than insurance-based care. Because services are not billed through insurance, your visit details and diagnostic codes are not shared with insurance companies.

Billing Information

Membership billing occurs on the first day of each month unless otherwise stated in advance. If you enroll in a membership after the first of the month, you will be charged the full membership fee for that month once you have signed all necessary paperwork to begin your membership access. Membership fees are not prorated based on enrollment or service start date. Please keep in mind that delays in completing application documentation may result in the cancellation of your membership application.

What happens if I stop paying my monthly fee?

A payment lapse occurs if a payment fails and the member does not respond within seven days. Once it is considered a lapse, there is a seventy-five dollar restart fee to rejoin the membership.

 

Can I cancel whenever I want?

Members must complete the six-month minimum commitment. After that period, you can choose whether to continue or discontinue the membership.

 

What happens if Radical Healing ends the pilot early?

If the program is discontinued before the six-month period ends, Radical Healing will issue a prorated refund for any unused portion of the membership.

 

Is there a limit to how many people can join?

Yes. Each plan has a participant cap to ensure that members receive dependable access and avoid long wait times during this pilot program.

Can I use my insurance at the same time?

You can still hold insurance, however, you must agree to waive insurance for the service of membership you are signing up for. 

What if I have questions about billing or membership issues?

For non-urgent concerns, members can use the designated membership form or email membership@radicalhealing.us. Responses are provided within seventy-two business hours unless otherwise stated.

How do I schedule visits?

Therapy appointments are booked directly with your provider or through the established scheduling channels. Ensure that you identify yourself as a member when scheduling so you are accurately billed.
